Pregnant Women.

Breastfeeding Women up to one year after baby’s birth.

Postpartum (non breastfeeding) women up to six months after baby’s birth.

Infants.

Children up to five years of age.

What documents do I need to bring?

Proof of Identity Driver’s License, CIB, or a current shot record.

Proof of Physical Address: utility bill or rental agreement.

Proof of Pregnancy.

Proof of Medicaid, SNAP, TANIF or FDPIR.

(Only if participant does not receive Medicaid, SNAP, TANIF or FDPIR)

Check stubs, social security, unemployment, self-employment, contribution of loans statement.
